New Clients: Register for your Patient Portal and complete your intake forms
To simplify the check-in process your clinician’s intake forms are available in digital format so you can complete and submit them online. If it is not possible for you to do your forms on-line, please let us know. We are happy to find other ways to help with this.
During your intake call, you were emailed a link to our electronic health record system, Therapy Appointment, to begin your patient portal registration process. Please check your spam folder in case your email provider sent the email to your spam folder rather than your inbox. The link sent to you is only valid for 24-48 hours so please be sure to complete your portal registration process as soon as possible. If your link expires, please call us right away so we can send you a new one.
Once you complete your registration process, you will find your intake forms ready for you under your “Documents” section. Please be sure to complete your intake forms at least 48 hours prior to your intake appointment so your provider has time to review them in preparation for your appointment. Because of how important this preparation is, most providers will cancel your intake appointment if the forms are not completed 48 hours prior to your appointment.
We know life happens and things are easily forgotten so we will attempt to reach you to remind you to do your forms if we see they are not getting done. If you need to reschedule your first appointment, please reach out and let us know. We are happy to facilitate a reschedule – and will offer this if your intake gets cancelled by your provider as well. Our goal is to have you come to your first appointment with everyone ready to go!

Current Clients: Access your medical record to update your information, manage your appointments, and communicate with your therapist:
If you are a current client of a therapist working out of the Bluestone offices and have already completed the registration process for your patient portal, you have the ability to access your chart through our electronic health records system, called Therapy Appointment. You may complete forms your provider may assign to you, exchange encrypted emails with your therapist, update your chart information (such as change of address or insurance), keep your credit card (including FSA or HSA cards) up to date, and manage your appointments.
Simply locate and click on your therapist’s name below. You will be directed to his or her account where you can access your chart. Please be sure you have your login name and password nearby as you will need these to gain entry.
